The Hospitals and Agencies Analyze page has multiple sections and tables that contain a spread of useful metrics and insights about your selected hospital of agency. The purpose of this article is to provide summaries of the different sections and tables. In addition, it is a hub with links to additional, more detailed content on each section and table.
As you identify metrics that stand out, you can make notes for later reference in the Provider Information section at the top of the page by clicking on the pencil icon.
Page Summary
The table below summarizes each of the sections and tabs in the Facility Analyze page. The section/tab names in the left column are links to the full article for that section/tab.
This section includes basic information on the selected Provider; specialty, location of practice, etc. In addition, there are internal notes, assigned users and Key Staff.
This section has several tables of metrics related to the selected facility and patient care post discharge.
Discharge Events by Setting - This table allows you to trace the flow of patients from facility discharge to post-acute admission and allows you to evaluate patient adherence to discharge coding.
Outcomes by Setting: Inpatient - For the different populations of patients from you selected facility to post-acute care, readmission rates and mortality rates are calculated for the different post-acute destinations with county and state averages available as benchmarks for comparison.
Patients Discharged with SNF Instructions (Inpatient only) - This table allows you to compare the impact of SNF care. All patients coded for SNF at discharge are compared between two groups - those who were admitted to SNF and those who weren't.
Patient Diagnostic Mix - breaks down the selected facility's patients by the principle diagnosis indicated on the claim and provides metrics for trended patient counts, SNF Utilization, Average Length of Stay, and Readmission rates with state and county comparisons.
Destinations
Two tables show the home health agencies and SNFs that admitted patients treated by the selected facility.
